Earth-5556[]
Despite the fact that Fenris, the Hell-Bringer travelled back in time to prevent the people of Gallifrey from becoming "Lords of Time," Rassilon was able to overcome this evil time traveller. Upon defeating him, Fenris tried to escape by activating his time jumper, but since he was separated from his Directional Control Device, he fell through the timestream in no particular direction, causing him to be lost in time.
It turned out that the Time Lords had yet discovered the secret to navigating the time stream, and so fortunately the device that Fenris left behind was just what they needed.
